LOCALLY DRIVEN POPULATION HEALTH MODEL OVERVIEW

This funding is for delivery organizations to implement targeted, locally informed, culturally responsible interventions to improve healthcare access and support referrals and access to other social supports. These activities will enable upstream and proactive health care for communities that have unmet health care needs.
